Grainy image off and on

What is going on with the image quality with one of my IPC-T5442T-ZE cameras? Not even a month old. When I am viewing the live feed on my monitor the image quality will change to this grainy one then snap back to the normal quality. See attached samples. The camera is being viewed straight from the NVR

What bitrate are you running and do you have h.265, AI or Smart Codec, or ROI turned on?
 
Is it only seen in live view? Or is this like that on the recorded video? Are you using substreams? Is it just when you click from a group view to the single camera view? I get that as a jump when I switch from a group view to a single cam view. But it is not like that in the recorded videos.
